Currently, in 2018, there is no specific Current Procedural Terminology (CPT®) code to report chronic care management of at least 30 minutes per calendar month that is provided personally by a physician or qualified health care professional (QHP). For 2019, new code 99491 should be used to report the chronic care management of at least 30 minutes per calendar month with the establishment, implementation, revision, or monitoring of a comprehensive care plan provided personally by a physician/QHP.
