Coding Guidelines
1. The guidelines of the Correct Coding Initiative (CCI) supersede all coding instructions in this LCD.
2. Low Level Laser Therapy should be coded using CPT code 97039. "Low Level Laser Therapy" should be entered in Item 19 of the CMS-1500 form or its electronic equivalent, if submitting electronically. Submission of claims for this service is only for the purpose of receiving a denial from Medicare, as this is a non-reimbursable service.
3. Claims for Low Level Laser Therapy (LLLT) that are being submitted for denial purposes should be submitted with modifier GA if a waiver of liability was signed by the beneficiary, or with modifier GZ if a waiver of liability was not obtained from the beneficiary.
4. Since this service is not a payable service, it is inappropriate to bill Medicare for it using any CPT code except 97039.
