There is much misunderstanding about the proper usage of CPT codes 92980, 92982, and 92984. CPT code 92980 is for a transcatheter placement of an intracoronary stent(s), percutaneous, with or without other therapeutic intervention, any method; single vessel. Thus, this service includes an angioplasty, as well as the stent placement. CPT code 92982 is for a percutaneous transluminal coronary balloon angioplasty; single vessel.
One may not bill for both a stent placement (92980) and an angioplasty (92982) on the same vessel. The payment for 92980 is about 35% more than for an angioplasty (92982). This reflects both the work involved in the angioplasty and work involved in the stent.
If an additional coronary vessel receives an angioplasty (besides the stented vessel), the proper code for the second angioplasty is 92984, not 92982. CPT 92984 is for a percutaneous transluminal coronary balloon angioplasty, each additional vessel. The stented vessel (92980) had an angioplasty and thus the proper code for a second angioplasty is 92984.
