When reporting drugs with a revenue code 0636 on a UB-92, the instructions are as follows: “Revenue code 0636 relates to HCPCS code, so HCPCS is the recommended code to be used in FL 44. The specified units of service to be reported are to be in hundreds (100s) rounded to the nearest hundred (no decimal).” Should the specified units of service be completed with the dosage amount given to the patient or by the number of units given based on the HCPCS definition of that drug?  ...
