Effective October 1, 2008, revisions have been made to the fifth-digit subclassification for the following categories: 203 Multiple myeloma and immunoproliferative neoplasms 204 Lymphoid leukemia 205 Myeloid leukemia 206 Monocytic leukemia 207 Other specified leukemia 208 Leukemia of unspecified type The fifth digit subclassification has been revised as follows: Fifth-digit “0” has been revised to state “without mention of having achieved remission” and to include “failed remission.” Failed remission refers to remission induction therapy where the patient is given a course of chemotherapy...
