Catheter/Snare Retrieval of Broken Guide Wire

The patient is a 96-year-old female with severe coronary artery disease. She had an angioplasty and attempts were made to pass a stent into the right coronary artery. During the manipulation of the wire, the wire broke off and was stuck in the right coronary artery (RV branch) and in the aorta. A micro-snare was used to catch the wire and the wire was then pulled out using the micro-snare catheter. The physician documented, “There was a broken piece of the wire in the right coronary artery and flapping in the aorta, which was successfully retrieved.” How should the diagnosis for the broken wire and the procedure to remove it be coded? ...
