CMS issues a clarification of brachytherapy source descriptors

The descriptors published in Transmittal 423 for HCPCS codes C2634 and C2635 are incorrect. In Table 40 of the November 15, 2004 final rule, the correct descriptors are listed. In a recent transmittal, Transmittal 514, CMS is reinstating the correct long descriptors for HCPCS codes C2634 and C2635, as follows: C2634—Brachytherapy Source, High Activity, Iodine-125, greater than 1.01 Mci (NIST), per source C2635—Brachytherapy Source, High Activity, Paladium-103, greater than 2.2 MCI (NIST), per source Additional information regarding these brachytherapy sources can be found in Transmittal 423. ...
