Correction to Medicare Coverage Indicator

The following table lists the albuterol drug codes that were listed incorrectly on the January 2009 HCPCS code set file. The July 2009 quarterly update corrected the file to indicate that these codes have “D” coverage, which indicates that these codes have special coverage instructions for Medicare reporting. These special coverage instructions can be found in the local coverage determinations (LCD). These changes took effect for claims of date of service on or after April 1, 2008. See Table 4. Table 4.
