CPT code 22845

There seems to be some confusion regarding the use of 22845, Anterior instrumentation; 2 to 3 vertebral segments. However, CPT code descriptor, coding instructions and examples, appear to indicate that this code should be reported in addition to other primary procedure(s) which do not involve scoliosis correction or lumbar spine only. I understand if the instrumentation is part of a device (i.e. synthetic cage), the screws and plates are included and not reported separately. Can CPT code 22845 be reported for other conditions besides scoliosis or the lumbar spine? ...
