A patient with history of amputation of the right 2nd, 3rd and 4th toes was admitted with ascending cellulitis and infection of the amputation site. Progress notes indicate a sharp scalpel debridement of necrotic skin and tissue at the amputation site was performed at bedside on the third day of hospitalization. On day six of the hospitalization, the documentation shows a minor nonexcisional debridement was performed. What are the diagnosis and procedure code assignments for this admission?
