Endoscopic retrograde cholangiopancreatography with balloon dilation

Patient with right upper quadrant pain and mildly elevated transaminases presents for an endoscopic retrograde cholangiopancreatography (ERCP) with balloon dilation of the biliary and pancreatic ducts. During the ERCP evidence of a gastric bypass was seen. However, the gastro-jejunal anastomosis was normal and once the jejuno-jeunal anastomotic site was reached a tattoo was injected. This area was identified as intubation was difficult due to the angle and looping at the anastomotic site. Both biliary and pancreatic ducts were successfully cannulated.
