Never Event National Coverage Determination (NCD) Modifiers for Hospital Outpatient Reporting

Never events are clearly identifiable and preventable errors that occur in medical care and can have serious consequences for patients. To identify these occurrences a list of national coverage determination (NCD) modifiers were provided by CMS. Hospital outpatient departments are now required to append the appropriate NCD modifier to all lines of service related to the erroneous surgery(s) performed.
