Flat Back Syndrome

A 52-year-old man had a 30-plus year history of Harrington rod fusion for scoliosis that has resulted in adjacent segment degeneration at L2-L3 with flat back deformity. Two years ago the patient had an L4-L5 and L5-S1 anterior interbody fusion followed by posterior fusion connecting to the Harrington rod at L3-L5 and at S1. The patient now complains of low thoracic and lumbar level pain and needs a walker to support his forward-pitched gait due to sagittal imbalance and flat back syndrome with continued scoliosis.
