Retained Laparotomy Sponge during Cesarean Delivery

On page 14 of the First Quarter 2014 issue Coding Clinic for ICD-9-CM, advice was provided regarding the coding of a retained laparotomy sponge during Cesarean delivery. Upon further review of the case and the submitted operative report, the patient had remained in the operative room following emergent cesarean section when the patient was reopened to remove a retained laparotomy sponge. Therefore, do not assign code 998.4, Foreign body accidentally left during a procedure, as the sponge was retrieved before surgery ended.  This advice is consistent with advice previously published in Coding Clinic, First Quarter 2013, page...
