Integrated healthcare is a coordinated approach between Primary Care Providers and Behavioral health providers. This program is to detect, treat and ensure follow up for mental and physical conditions.
The system was created to integrate and share information with an interdisciplinary health care team to create better continuity of care. Primary care is usually the first place someone will go for help, therefore people who cannot or will not access behavioral healthcare services can be reached. This will provide an opportunity for early prevention and an opportunity to intervene, resulting in a better quality of life. It is believed that many behavioral and physical disorders are co-occurring, mainly in depression and chronic medical conditions.
