Employees need to be responsible for obtaining current and correct data. They should also understand that claims with missing or inaccurate patient information are delayed or denied. Some policy examples are:

• Information for the subscriber(s) of the insurance coverage should also be obtained.

• A photocopy of both front and back of the patient’s health plan identification card should be kept on file and updated when the policy renews.
