There are numerous types of behavioral health screening tools for a variety of healthcare providers. Some, like the AUDIT and SBIRT are especially applicable to primary care providers caring for Medicare beneficiaries.

AUDIT - Alcohol Use Disorders Identification Test
10 Question survey - can be done as interview or as patient completed questionnaire.
DAST - Drug Abuse Screening Test
20 Question survey focused primarily on drugs instead of alcohol.
MAST - Michigan Alcoholism Screening Test
25 Question survey for greater details and quantitative data. CLICK HERE to download the questionnaire.
MSE - Mental Status Exam
To be used with codes .... or as patient completed questionnaire.
SBIRT - Screening, Brief Intervention, and Referral to Treatment
Coverage of the set types of services varies by payer. Some types of screenings are covered as preventative services, so check with payers to determine their policies.
