Percentage of patients aged 18 years and older who have experienced an acute brain injury (ischemic stroke, hemorrhagic stroke, acute brain injury), seen for an office visit during the measurement period who were screened for depression using a validated tool and if screened positive AND if positive, a follow up plan of care is documented on the date of the positive screen
