Percentage of patients diagnosed with atrial fibrillation (afib) who are assessed for lifestyle and disease factors that contribute to uncontrolled atrial fibrillation at least once in the past 12 months. This measure is reported once per reporting period for each patient having one or more denominator qualifying visits.
