Percentage of actively enrolled home-based primary care and palliative care patients with a new diagnosis of altered mental status, delirium, or new or unexpected cognitive decline whose medication list was reviewed for offending medications and either: no potentially offending medications were identified, or potentially offending medications were identified and use was discontinued or continued use justified.
