Four rates are reported: Rate 1: Risk Assessment or Counseling for Alcohol Use, Rate 2: Risk Assessment or Counseling for Tobacco Use, Rate 3: Risk Assessment or Counseling for Other Substance Use, and Rate 4: Risk Assessment or Counseling for Sexual Activity.
