For patients with terminal conditions, physicians shall provide advance care planning to identify a patient's end-of-life treatment preferences and submit a new physician's order for life sustaining treatment form ("POLST"; also known as "MOST", "MOLST", or "POST" in some states) into an ePOLST Registry, making it accessible across the continuum of care.
