AHA Coding Clinic® for HCPCS - 2013 Issue 4; ASK THE EDITOR
ACell porcine xenograft
A patient is seen at our facility one-week postop debridement of the medial cuneiform bone of the right foot with application of ACell porcine xenograft. The left foot is one-week status post CelluTome keratinocyte grafting. The dressings on both the right and left foot were removed and there was no clinical evidence of infection. During this encounter, the wounds were dressed and two small sheets of ACell graft (porcine xenograft) were placed over the wound on the right foot. On the left foot, porcine graft was sprinkled over the wound and covered with Adaptic followed by Acticoat Flex. In this case, would the sprinkling of porcine graft on the left foot be appropriately coded to a skin substitute graft? We believe the graft material is an ACell matristem micromatrix product, that codes to HCPCS code Q4118, Matristem micromatrix, 1 mg, and CPT code 15275, Application of skin substitute graft to face, scalp, eyelids, mouth, neck, ears, orbits, genitalia, hands, feet, and/or multiple digits, total wound surface area up to 100 sq cm; first 25 sq cm or less wound surface area. ...

Article Overview
This premium article focuses on coding questions that arise when porcine xenograft products are used during wound care after debridement and grafting procedures. It is intended for coding professionals and reimbursement staff who need to understand the general documentation and code-application issues discussed for skin substitute products and related HCPCS/CPT reporting.
Why This Topic Matters
Skin substitute and wound graft coding can be difficult when product type, application method, and anatomic site all affect reporting. This article helps readers evaluate whether the documented service aligns with the coding framework discussed in the piece.
