Basivertebral nerve ablation

Patient with lumbosacral degenerative disc disease presents for basivertebral nerve ablation. A small incision was made and under fluoroscopic guidance the introducer trocar is inserted percutaneously into the pedicle and the L5 vertebral body. A cannula is used to penetrate the vertebral body and a stylet is inserted (midline placement) creating a channel within the vertebral body in order to access the basivertebral nerve. A thermocouple electrode is placed posterior to the vertebral wall in the middle of the vertebral body, connected to the generator, and heat is applied for 15 minutes to ablate the basivertebral nerve.
