Cricopharyngeal Muscle Botox Injection
To treat cricopharyngeal spasm and retrograde cricopharyngeal dysfunction, the patient elected to undergo direct pharyngolaryngoscopy with botulinum toxin injection into the cricopharyngeal muscle. An operating laryngoscope was introduced transorally to the upper esophageal sphincter, and the patient was suspended from the Mayo stand using the Lewy arm. The hypopharynx and esophagus were visualized with a rigid straight laryngeal telescope.The cricopharyngeal sling was visualized distal to the laryngoscope. Under telescopic guidance, diluted botulinum toxin type A solution was injected premucosally into the cricopharyngeal muscle at the 6, 5, and 7 o’clock positions. A total of approximately 1.0 mL (66 units) was delivered in three divided aliquots.The treatment site was then treated with a topical cocaine solution applied on a pledget.
