Deletions and changes for several HCPCS/CPT codes for drugs, biologicals, and radiopharmaceuticals

As of January 1, 2016, several HCPCS C-codes for certain drugs, biologicals, and radiopharmaceuticals were deleted and replaced by permanent HCPCS codes. For many other HCPCS/CPT codes for drugs, biologicals, and radiopharmaceuticals changes were made to their code descriptors. A crosswalk of the deleted HCPCS C-codes and their replacements are listed in the table as well.
