A patient with urethral diverticulum presented for excision of the diverticulum with rotation of Martius flap.
The diverticulum was excised from the urethra resulting in a defect in the ventral aspect of the urethra. This was over sewn and a longitudinal incision was made in the left labia. Fatty tissue was dissected medially and laterally to define a flap which could be rotated medially. A passage was created below the vaginal mucosa on the left side. The flap was able to be brought up through the defect and superimposed under the urethra. What is the CPT code for the Martius flap?
Assign CPT code 20926, Tissue grafts, other (eg, paratenon, fat, dermis), for the Martius flap in addition to CPT code 53230, Excision of urethral diverticulum (separate procedure); female, for the diverticulectomy. Since code 53230 is designated as a separate procedure, append Modifier 59, Distinct procedural service.
