Modifier additions, reactivations, and deletions

In Transmittal 514, CMS has provided a list of modifiers that were recently reactivated or deleted. The modifiers were identified as follows: Modifier SS—Home infusion services provided in the infusion suite, reactivated effective October 1, 2004. Modifier GX—Service not covered by Medicare, reactivated effective January 1, 2005. Modifier QR—Repeat clinical diagnostic laboratory test performed on the same day to obtain subsequent reportable test value(s), was added to the list of valid modifiers effective January 1, 2005 Modifier CG—Innovator drug dispensed, deleted from the list of valid modifiers effective April 01, 2004 ...
