Modifier additions, reactivations, and deletions
In Transmittal 514, CMS has provided a list of modifiers that were recently reactivated or deleted. The modifiers were identified as follows: Modifier SS—Home infusion services provided in the infusion suite, reactivated effective October 1, 2004. Modifier GX—Service not covered by Medicare, reactivated effective January 1, 2005.
