Transurethral Resection of Bladder Tumor – Procedure 1
A patient presents for transurethral resection of bladder tumor (TURBT) for extensive papillary urothelial carcinoma of the bladder. A rigid cystoscope with a 26 Fr continuous sheath and visual obturator was inserted into the urethra and advanced towards the bladder. A bilobar obstructive prostate was noted at the prostatic urethra with papillary appearing tumor circumferentially. An extensive tumor (>7 cm), expanded from the right lateral wall to the right bladder dome; that included a posterior wall tumor (2-3 cm) that expanded from the right lateral wall towards the right bladder dome (5 cm). A tumor spanned the entire bladder wall, and the left anterolateral wall contains tumorous (>5 cm) bladder-wall pouches (saccules). The tumors were resected with a bipolar loop and subsequently, the bladder was vaporized with a button electrode. It was decided the procedure needed to be staged as residual tumor was left at the bladder dome and prostatic urethra.The entire tumor was not resected due to its size.
