Accidental Durotomy (Dural Tear)

Effective October 1, 2008, new codes have been created for intraoperative incidental/inadvertent dural tear (349.31) and other dural tear (349.39) in order to distinguish dural tears from other types of accidental surgical lacerations. The dura mater covers the spinal cord and the spinal nerves. A tear in the dura that occurs during spinal surgery is not unusual and is typically repaired intra-operatively when identified. Primary closure of the dural tear is usually accomplished. Dural tears that are not discovered during surgery can result in leakage of cerebrospinal fluid (CSF), leading to CSF headache, caudal displacement of the...

Article Overview

This article covers a coding update that distinguishes different types of dural tears and places the topic in the context of spinal surgery and nervous system disorder classification. It is intended for coders and other revenue cycle professionals who need to understand the scope of the change, the terminology used in clinical documentation, and the related coding references discussed in the article.

Why This Topic Matters

Accurate identification of dural tear-related documentation affects diagnosis coding consistency and supports clearer reporting of intraoperative complications and related spinal procedure records.

Article Sections

  1. Overview of dural tear coding

Introduces the topic and explains why dural tears are being addressed as a distinct coding subject. It places the discussion within broader nervous system disorder classification.

  2. Coding Clinic reference

References guidance from Coding Clinic and summarizes the clinical significance of dural tears as discussed in that source.

  3. Code category and subcategory updates

Lists the relevant diagnostic code category and newly created subcategory and codes associated with dural tear reporting.

  4. Clinical example and coding question

Presents a spinal surgery case scenario and frames the coding question raised by the documentation. The section continues into the coding discussion that follows.
