Attempted stenting of a coronary artery chronic total occlusion with drug-eluting stent insertion of a separate vessel

A patient with coronary artery chronic total occlusion (CTO) and stenosis presents for insertion of drug-eluting stents into the ramus branch and the mid-circumflex arteries. Local anesthesia was utilized. The right radial artery was prepped and a 7-French sheath was inserted. A run-through wire was advanced to the ramus branch and a 2.5 x 22 mm drug-eluting stent was deployed up to 16 atmospheres.  A microcatheter was advanced to the mid-circumflex artery initially, over the run-through wire and then a coated guidewire was used. Due to several attempts to cross the total occlusion, the procedure was discontinued. The physician made the decision to treat the patient medically. What is the correct CPT code(s) and modifier(s) for a successful PTCA with drug-eluting stent insertion into the ramus artery? What is the correct code(s) and modifier(s) for the attempts to place the stent into the mid-circumflex artery? ...

Article Overview

This article explains how to think about coding a coronary intervention scenario that includes both a completed stent placement in one vessel and unsuccessful attempts to treat a separate chronic total occlusion. It is aimed at cardiology coders, outpatient facility coders, and anyone needing to distinguish completed work from attempted work in coronary intervention reporting. The article also references Medicare facility reporting and discusses the relevant CPT and HCPCS Level II code sets in the context of the scenario.

Why This Topic Matters

Mixed-success coronary procedures can affect both professional and facility reporting, so accurate code assignment depends on recognizing what was completed versus what was only attempted. This kind of guidance is especially important for compliant reporting in interventional cardiology cases.

Article Sections

  1. Case Scenario

A coronary intervention case involving treatment of separate coronary vessels is presented, including one completed service and one aborted attempt during the same encounter.

  2. Coding Questions

The article poses questions about professional coding, related modifiers, and facility reporting for the services described in the case.
