A patient with left fusiform dissecting aneurysm of the mid to distal V4 segment of the vertebral artery underwent placement of two pipeline shield devices. The first was deployed via microcatheter into the vertebral artery, proximal to the left posterior inferior cerebellar artery (PICA), with inadequate coverage of the dissecting aneurysm. The second was then deployed completely covering the previous pipeline device, slightly more distal and extending slightly more proximal but not covering the left PICA. Follow-up angiogram showed adequate coverage and increased stasis within the lumen of the dissecting aneurysm.
