Carotid Artery Pseudoaneurysm Embolization Using Flow Diverter Stent and Coils

A patient with a pseudoaneurysm of the right internal carotid artery (ICA) presented for endovascular embolization. Following percutaneous access, intracranial views confirmed pseudoaneurysm of the cervical right internal carotid artery extending into the petrous (intracranial) segment of the artery. A duo microcatheter was placed into the pseudoaneurysm for coil delivery. Through a second catheter, a Surpass® Flow Diverter was deployed covering the entry into the pseudoaneurysm. Then, three coils were successfully deployed into the pseudoaneurysm to facilitate thrombosis. AP and lateral projections demonstrated coil mass within the pseudoaneurysm facilitating contrast stasis and adequate coverage of the pseudoaneurysm neck by the flow diverter. When a flow diverter is used in combination with coil embolization, is the flow diverter considered an adjunct device and not coded separately? What ICD-10-PCS codes are assigned in this case? ...

Article Overview

This premium article reviews a vascular interventional case involving endovascular management of a pseudoaneurysm in the carotid circulation. It is aimed at coders, auditors, and CDI professionals who need to understand how the procedure is documented and how ICD-10-PCS assignment is approached when multiple embolization technologies are used together. The discussion focuses on general coding interpretation for the procedure and the broader issues of device reporting and procedure classification.

Why This Topic Matters

Carotid and intracranial endovascular procedures can be challenging to translate from operative documentation into ICD-10-PCS. Understanding how the procedure is classified helps support accurate inpatient coding, compliance, and consistency in reporting complex vascular interventions.

What You Will Learn

  • The clinical setting and procedural context for a carotid pseudoaneurysm embolization case
  • How the article frames ICD-10-PCS assignment for a complex endovascular embolization encounter
  • General coding considerations when multiple embolic devices are used in the same procedure
  • How the case relates to device classification within inpatient procedure coding

Who Should Read This

  • Inpatient coders
  • Coding auditors
  • Clinical documentation improvement specialists
  • Health information management professionals
  • Interventional radiology coding staff
