AHA Coding Clinic® for ICD-9 - 1991 Second Quarter
Cellulitis of the Skin
Cellulitis is an acute, diffuse infection of the skin and soft tissues that commonly results from a break in the skin, such as a puncture wound, laceration, or ulcer. Occasionally the break is so small that it cannot be identified by either the patient or the examining physician. Cellulitis of the skin is classified to category 681, Cellulitis and abscess of finger and toe, and category 682, Other cellulitis and abscess. An additional code should be assigned to indicate the organism when this information is available. Clinically, cellulitis usually presents as an abrupt onset of redness, swelling, pain, or...

Article Overview
This article is a coding guidance summary focused on cellulitis and related infection presentations. It discusses how the condition is classified, when additional diagnosis coding may be needed, and how related wound, ulcer, postoperative, burn, frostbite, and obstetric situations are handled in a general coding context. It is intended for coders, billers, and compliance staff who need to understand the scope of cellulitis-related code assignment and documentation considerations.
Why This Topic Matters
Cellulitis can affect diagnosis coding across multiple clinical settings, and related infection scenarios may involve more than one code set or chapter. Understanding the article helps readers recognize when cellulitis is addressed as part of a broader injury, wound, postoperative, or obstetric record.
Article Sections
-
General classification and documentation considerations
Introduces cellulitis as a skin and soft tissue infection and summarizes documentation and classification considerations. It also addresses the relationship between cellulitis and other findings that may appear in wound documentation.
-
Cellulitis with wounds, ulcers, and related injuries
Covers coding considerations for cellulitis occurring with open wounds, ulcers, superficial injury, burn, frostbite, and gangrenous presentations. This section also includes examples illustrating the general sequencing context discussed in the article.
-
Postoperative and IV-related cellulitis
Discusses cellulitis associated with postoperative wounds and skin penetration related to IV therapy. It includes a general note on postoperative infection coding and an example involving an operative wound.
-
Cellulitis in other anatomical and obstetric contexts
Addresses cellulitis when it appears outside the skin and subcutaneous tissue, including orbital, tonsillar, pelvic, and obstetric-related contexts. It also notes that some of these situations follow other chapter-based coding guidance.
