Cerebrovascular Accident (Stroke)

Effective October 1, 2004, a change has been made in the classification of unspecified cerebrovascular accident (CVA) or stroke. Prior to this change, these conditions were indexed to code 436, Acute, but ill-defined, cerebrovascular disease. Strokes or CVAs can be grouped as hemorrhagic or nonhemorrhagic. In many instances, the documentation available does not specify whether the stroke or CVA is hemorrhagic or nonhemorrhagic. The majority of strokes are nonhemorrhagic. Physicians routinely equate the terms stroke and CVA with cerebral infarction, and yet in the absence of additional documentation, these conditions were being grouped under code 436. To allow for...

Article Overview

This article covers an ICD-9-CM coding update involving unspecified cerebrovascular accident or stroke, including how the classification changed and how related stroke terminology is organized. It is intended for coders, auditors, and clinical documentation staff who need to understand the scope of the update and the broader categories of cerebrovascular conditions referenced in the guidance.

Why This Topic Matters

Stroke terminology is often used inconsistently in clinical documentation, so understanding the classification update helps support more consistent record abstraction, reporting, and code lookup in ICD-9-CM-based materials.

Article Sections

  1. Classification update and terminology changes

Summarizes the change in how unspecified cerebrovascular accident or stroke is indexed and the related adjustments to the code classification structure.

  2. Stroke categories and unspecified documentation

Reviews the broad hemorrhagic and nonhemorrhagic stroke groupings and the documentation issue that can leave stroke type unspecified.

  3. Related indexing and exclusion guidance

Covers the indexing changes and the related exclusion list for broader cerebrovascular terminology within the classification.

  4. Clinical examples

Presents sample hospital documentation scenarios used to illustrate the coding topic in a general way.
