Closure of Paravalvular Leak Using Amplatzer® Vascular Plug
The patient, who is status post-surgical aortic valve replacement, underwent closure of an aortic paravalvular leak in the hybrid catheterization lab. The provider documented that a guide catheter was used to locate the paravalvular defect, and a glide wire was traversed across the paravalvular leak. Using transesophageal echocardiography, an Amplatzer® vascular plug (AVP) was deployed with reduction in the paravalvular leak. A second AVP device was placed across the smaller defect. ICD-10-PCS table 02W, Revision, heart and great vessels, appears to be the correct table, with “aortic valve” assigned as the body part (4th character). However, “percutaneous” is not an option for the approach (5th character). Code 02UF3JZ, Supplement aortic valve with synthetic substitute, percutaneous approach, also appears to be an option. What is the correct ICD-10-PCS code assignment for closure of a perivalvular leak using Amplatzer® device? ...

Article Overview
This premium article focuses on coding considerations for a catheter-based closure of an aortic paravalvular leak in the setting of prior surgical aortic valve replacement. It is aimed at coders, auditors, and clinical documentation specialists who work with cardiovascular procedures and ICD-10-PCS assignment. The article addresses how the procedure is described in the operative record and discusses related ICD-10-PCS table and code selection context.
Why This Topic Matters
Accurate classification of cardiac repair procedures depends on matching the documentation to the correct body system, body part, approach, and device details. This article helps readers understand the coding analysis for a complex structural heart procedure.
