Curettage of Bilateral Humeral Head and Bone Graft Placement

The patient is a 16-year-old who is admitted for surgical treatment of avascular necrosis (AVN) of both humeral heads, secondary to sickle cell vaso-occlusive disease. He underwent curettage and bone graft of both humeral heads. During the procedure, a C-arm was utilized to identify the lesion on the left upper extremity where an incision was made with dissection down to the periosteum. The periosteum was incised; a guidewire was drilled into the lesion of the humeral head; and an expandable reamer umbrella was opened to core out the lesion. The reamer was then withdrawn leaving the protection sleeve in place, Pro-dense™ bone graft was mixed and injected via cannula into the lesion. The procedure was then repeated on the right. What are the appropriate root operations for curettage and Pro-dense™ bone grafts to treat AVN of the bilateral humeral heads? ...

Article Overview

This premium article reviews inpatient procedure coding for a surgical case involving avascular necrosis of both humeral heads in a pediatric patient with sickle cell disease. It focuses on the operative approach, the general coding framework for the treatment performed, and the root operations involved in the procedure. The article is intended for coders, CDI specialists, and revenue cycle staff working with orthopedic and musculoskeletal inpatient cases.

Why This Topic Matters

Bilateral orthopedic procedures can be coded differently from unilateral cases, and accurate identification of the procedure framework is important for compliant inpatient coding and reporting. This article helps readers understand the coding considerations for a complex musculoskeletal surgery without relying on outside assumptions.

What You Will Learn

  • How this bilateral humeral head procedure is framed for inpatient procedure coding
  • Which general ICD-10-PCS concepts are relevant to lesion treatment and graft placement
  • How operative details factor into identifying the procedure structure for coding review
  • Why bilateral musculoskeletal cases require careful code assignment review

Who Should Read This
