AHA Coding Clinic® for ICD-9 - 1991 Third Quarter
Diabetes Mellitus and Related Conditions
Diabetes mellitus is a disorder of glucose metabolism due to either an absolute decrease in the amount of insulin secreted by the pancreas or to a reduction in the biologic effectiveness of the insulin secreted. It is classified into two major categories: Type I or insulin-dependent diabetes mellitus (IDDM), also known as "Juvenile-onset diabetes mellitus," and Type II or noninsulin-dependent diabetes mellitus (NIDDM), also known as "adult-onset diabetes mellitus." These two groups are indicated in the classification scheme by the fifth digit code. Other conditions frequently confused with true diabetes mellitus include impaired glucose tolerance and gestational...

Article Overview
This article explains the scope of diabetes mellitus and related conditions in ICD-9-CM, including major diabetes groupings, abnormal glucose tolerance, pregnancy-related conditions, neonatal diabetes, secondary diabetes, and complication classification. It is intended for coders, auditors, and revenue cycle staff who need to understand how the topic is organized in the coding system and what types of guidance the article addresses.
Why This Topic Matters
Diabetes-related diagnoses are common and often complex because similar clinical situations may fall into different code categories. Understanding the article helps users identify the broad coding areas covered and the kinds of classification guidance discussed for ICD-9-CM diabetes topics.
Article Sections
-
Overview of diabetes and related conditions
Introduces diabetes mellitus as a category within ICD-9-CM and describes related conditions that are commonly discussed alongside it. The section frames the article’s focus on classification rather than clinical management.
-
ICD-9-CM coding guidelines for diabetes mellitus and abnormal glucose tolerance
Presents the main guideline areas addressed in the article, including subclassification topics and related diabetes categories. The section serves as a roadmap to the coding subjects that follow.
-
Category 250: Diabetes Mellitus
Covers the structure of the principal diabetes category and its subtype organization. The section discusses how the classification framework distinguishes major diabetes groupings.
-
Impaired Glucose Tolerance
Addresses abnormal glucose tolerance as a related topic and notes its placement in the coding structure. The section explains how this condition is treated as separate from diabetes mellitus in the article.
-
Diabetes Mellitus Complicating Pregnancy
Discusses diabetes in the setting of pregnancy and the associated classification topic. The section also covers related pregnancy-associated glucose tolerance issues and infant-related conditions.
-
Secondary Diabetes Mellitus
Reviews diabetes associated with underlying conditions or external causes and describes the related classification category. The section also touches on situations where another diagnosis drives the diabetes-related coding approach.
-
Complications of Diabetes Mellitus
Summarizes the article’s discussion of diabetes-related complications and how they are organized in the classification system. The section distinguishes broad complication groupings within the overall topic.
