Endoscopic elastography of the liver
A patient with cirrhosis and an abnormal abdominal/pelvic CT scan presented for esophagogastroduodenoscopy (EGD) with biopsy and an endoscopic ultrasound (EUS) elastography of the liver. Pressure measurements of the hepatic vascular anatomy were also performed. The endoscope was passed under direct vision through the mouth and advanced to the second part of the duodenum. The esophagus, stomach and duodenum were all normal. The EUS elastography of the liver was performed showing liver stiffness. Shear wave measurements were performed to identify the elasticity of the tissue, which was normal. The hepatic vasculature including the hepatic and portal veins were identified and pressure measurements were obtained. Lastly, an EUS-guided liver biopsy was also performed. What are the appropriate codes for the procedures performed? ...

Article Overview
This article explains a liver-focused endoscopic ultrasound procedure performed during an upper endoscopic exam and is aimed at coders working with gastroenterology, endoscopy, and biopsy services. It covers the clinical context, the procedural components involved, and the coding context for reporting the services documented in the encounter.
Why This Topic Matters
Accurate coding for combined endoscopic and ultrasound-based liver evaluation depends on recognizing which procedure components are documented and how they are grouped for reporting. This topic is relevant to coders and billers handling complex gastroenterology procedures, especially when multiple endoscopic services occur in a single encounter.
What You Will Learn
- How the documented endoscopic liver evaluation is described in the clinical note
- What types of procedure components are involved in an upper endoscopy with ultrasound-based liver assessment
- How the article frames coding considerations for a combined gastroenterology encounter
- What information in the note supports procedure reporting in a cirrhosis workup
Who Should Read This
