AHA Coding Clinic® for ICD-9 - 2010 Issue 4; VOLUMES 1 & 2 NEW/REVISED CODES
Incompatibility Reaction Due to Blood Transfusion
Effective October 1, 2010, subcategories 999.6, ABO incompatibility reaction, 999.7, Rh incompatibility reaction, and 999.8, Other infusion and transfusion reaction, were revised and expanded to create unique codes to identify the different types of hemolytic transfusion reactions (HTR). These new code sets include both acute hemolytic transfusion reaction and delayed hemolytic transfusion reaction and will allow data capture of the various HTRs. Additionally, a new code has been created to describe a febrile nonhemolytic transfusion reaction (780.66). A febrile nonhemolytic transfusion reaction (FNHTR) is a minor reaction that commonly occurs following the transfusion of blood...

Article Overview
This article covers changes to transfusion reaction coding in ICD-9-CM, focusing on revised and newly created categories for hemolytic transfusion reactions, Rh and non-ABO incompatibility, and febrile nonhemolytic transfusion reactions. It is relevant to coders, CDI staff, and compliance teams who work with transfusion-related diagnoses and need to understand how the code structure was expanded and reorganized.
Why This Topic Matters
Transfusion reactions can affect diagnosis coding, data capture, and reporting for blood bank and inpatient encounters. This article helps readers understand the scope of the ICD-9-CM revisions and the broader clinical categories addressed by the changes.
Article Sections
-
Overview of transfusion reaction code revisions
Introduces the ICD-9-CM changes made to transfusion reaction categories and the purpose of the revisions. Summarizes the general clinical areas affected by the update.
-
Clinical background on febrile nonhemolytic transfusion reaction
Describes the broad clinical features and timing of febrile nonhemolytic transfusion reaction. Explains why the condition is discussed separately within the transfusion reaction update.
-
Clinical background on hemolytic transfusion reactions
Provides an overview of hemolytic transfusion reactions and distinguishes acute and delayed presentations. Includes general discussion of compatibility-related causes and clinical impact.
-
Antibodies and reporting context
Lists broad antibody groupings associated with transfusion reactions and notes reporting context from federal blood safety surveillance. Connects the article to national monitoring of transfusion-related outcomes.
-
ICD-9-CM code updates and revised categories
Outlines the revised and newly created diagnosis categories included in the update. Covers the affected transfusion reaction groupings and related exclusions without providing coding instructions.
