AHA Coding Clinic® for ICD-9 - 2008 Issue 4; VOLUMES 1 AND 2 NEW/REVISED CODES
Infusion and Transfusion Reaction
Effective October 1, 2008, the code title has been revised and new codes have been created in subcategory 999.8, Other infusion and transfusion reaction. The change creates new codes to differentiate extravasation of vesicant chemotherapy (999.81), extravasation of other vesicant agent (999.82), other infusion reaction (999.88), and other transfusion reaction (999.89). Extravasation refers to the leakage of intravenous drugs from the vein into the surrounding tissue—sometimes also referred to as infiltration. Vesicants are chemically active substances that can produce blistering on direct contact with the skin or mucous membrane. The leakage...

Article Overview
This article explains updates to diagnosis coding for infusion and transfusion reactions and discusses the broader clinical situations covered by the revised subcategory. It is aimed at coders, billers, and compliance staff who need to recognize when a reaction belongs in this family of codes and understand the related documentation context. The article also references historical coding guidance and an example scenario involving chemotherapy-related infusion complications.
Why This Topic Matters
Accurate reporting of infusion and transfusion reaction diagnoses depends on recognizing the type of reaction, the setting in which it occurred, and whether related conditions are present. This topic matters because the article ties revision changes to practical coding review and documentation interpretation.
