Insertion of Two Impella® Devices During Separate Operative Episodes
A patient with cardiogenic shock and acute mitral valve insufficiency was transferred to our facility for Impella® insertion. The patient was taken to the catheterization lab immediately following admission. Following left groin access, an Impella® CP was successfully inserted. Four hours following completion of this procedure, the patient’s condition worsened and the patient was taken to a hybrid operating room. The right groin was accessed and an Impella® RP was successfully implanted. Since the Impella® devices were inserted into the same patient, would it be appropriate to report the insertion of a biventricular Impella® device? What are the appropriate code assignments for these procedures? ...

Article Overview
This Find-A-Code article addresses coding questions arising from separate operative episodes involving Impella device placement in a critically ill cardiac patient. It is aimed at coders and billing staff working with cardiac catheterization, mechanical circulatory support, and inpatient procedural coding, and it focuses on how to think about reporting device insertions when more than one procedure occurs during the encounter.
Why This Topic Matters
Properly identifying the procedural scope of mechanically assisted cardiac support matters for accurate hospital and physician coding, especially when interventions occur in more than one setting or at more than one time. The article helps readers understand how the scenario is framed for code assignment without relying on assumptions that could affect claim accuracy.
