Late Effect of Cerebrovascular Disease
Note: As of October 1, 2007, code V12.54 has been added for personal history of transient ischemic attack and cerebral infarction without residual effects. Fourth and fifth digits have been added to code 438, Late effects of cerebrovascular disease, to allow identification of specific types of deficits due to old cerebrovascular accidents (CVA). The intent is to improve data capture for these patients by linking the specific deficit with the old CVA. This will also allow the reporting of new sequela from a current stroke as well as the deficits from previous strokes. Cerebrovascular disease is a disorder...

Article Overview
This article covers coding guidance for late effects of cerebrovascular disease and related history-of-stroke situations. It discusses ICD-9-CM category 438 updates, the addition of history code V12.54, and broader guidance intended to improve documentation of residual deficits and sequelae after cerebrovascular events. The content is relevant to coding staff, CDI professionals, and clinicians involved in stroke-related record abstraction and rehabilitation coding.
Why This Topic Matters
Stroke aftercare and residual deficits are common sources of coding complexity, especially when a current condition, a prior event, and rehabilitation services are all present in the record. Understanding the structure and updates described in this article helps support more consistent reporting and data capture.
