AHA Coding Clinic® for ICD-9 - 2004 Issue 4; VOLUME 3 NEW/REVISED PROCEDURE CODES
Neurostimulator Pulse Generator
Effective October 1, 2004, new codes have been created to specifically identify insertion of single and dual array neurostimulator pulse generators (86.94-86.95) as well as the replacement of other neurostimulator pulse generator (86.96). There are three types of neurostimulators, intracranial, spinal, and peripheral. Neurostimulators consist of two key components: a pulse generator that is inserted into a subcutaneous pocket (under the skin of the abdomen or chest), and one or more leads that are connected to a generator, tunneled and placed at the target area. The pulse generator is equipped with a battery that will deplete...

Article Overview
This article reviews how neurostimulator pulse generator procedures were classified after new ICD-9-CM codes were introduced effective October 1, 2004. It is aimed at coding professionals who need to understand the general scope of neurostimulator-related procedure reporting, including the distinction between generator and lead components and the broader categories of intracranial, spinal, and peripheral systems.
Why This Topic Matters
Accurate classification of neurostimulator procedures depends on recognizing which component is being reported and whether the device is single, dual, or another type. The article helps readers orient to the relevant procedure coding framework and the kinds of documentation issues that affect code selection.
Article Sections
-
Overview of neurostimulator components and code changes
Introduces the major neurostimulator system types and describes the coding update that created more specific procedure classification. It also explains the general relationship between the pulse generator and lead components.
-
Code assignment by target area and device type
Discusses how procedure reporting is organized by the nervous system region being treated and by the device configuration. The section places the article’s coding discussion in the context of intracranial, spinal, and peripheral systems.
-
Prior ICD-9-CM classification
Summarizes the earlier approach used before the newer codes were introduced. It contrasts the older classification approach with the more detailed structure described in the article.
-
New codes 86.94, 86.95, and 86.96
Presents the new procedure code group discussed in the article and the general distinctions among the listed code categories. It also notes the relationship to associated lead implantation references.
-
Clinical example
Provides a short scenario involving generator replacement and asks for the appropriate code assignment. The example is used to illustrate the broader topic of neurostimulator pulse generator reporting.
