Novel Influenza A

Effective October 1, 2011, codes in subcategory 488.1, Influenza due to identified novel H1N1 influenza virus, have been retitled to state “Influenza due to identified 2009 H1N1 influenza virus.” The 2009 pandemic influenza is now regularly referred to as 2009 H1N1 influenza, rather than novel H1N1 influenza. Additionally, a new subcategory and new codes have been created to describe novel influenza A. The new codes include Influenza due to identified novel influenza A virus with pneumonia (488.81); Influenza due to identified novel influenza A virus with other respiratory manifestations (488.82)...

Article Overview

This article explains an ICD-9-CM update that revises influenza classification terminology and adds new diagnosis categories for novel influenza A. It is intended for coders, CDI professionals, and clinical documentation staff who need to understand the scope of the new terminology, the affected code groups, and the supporting public health context from CDC and NCIRD. The article also covers documentation and reporting guidance, including references to official coding guidelines and distinctions among related influenza categories.

Why This Topic Matters

Accurate influenza coding depends on recognizing the updated terminology, the revised categories, and the reporting framework described in the article. This matters for correct diagnosis assignment, consistent documentation review, and alignment with official coding guidance.

Article Sections

  1. Update overview and effective date

Summarizes the ICD-9-CM revision timeframe and the introduction of new terminology for novel influenza A and 2009 H1N1. It also identifies the organizations associated with the recommendation.

  2. Background on influenza viruses and novel influenza A

Provides general clinical and epidemiologic context about influenza virus types, subtypes, and seasonal circulation. It also describes how novel influenza A is distinguished from currently circulating human influenza viruses.

  3. Reporting and documentation guidance

Discusses reporting status and the documentation context associated with confirmed cases. It also references official coding guidance for handling qualified diagnostic statements.

  4. ICD-9-CM category and index revisions

Outlines the affected ICD-9-CM influenza categories and the related index and exclusion changes. This section focuses on the structure of the revisions rather than code-level instructions.

  5. New novel influenza A subcategory and codes

Describes the newly added subcategory for novel influenza A and the associated clinical groupings covered by the new codes. It also notes related exclusion references and supplemental pneumonia guidance.

  6. Indexing and documentation questions

Presents a coding question about how documentation wording relates to influenza terminology in the index. It also highlights ambiguity in physician documentation and the need for specificity.
