AHA Coding Clinic® for ICD-9 - 2010 Issue 1; New Code
Optical Coherence Tomography (OCT)
Effective October 1, 2009, two new codes were created (38.24-38.25) to describe intravascular imaging of coronary vessels and non-coronary vessels by optical coherence tomography (OCT). Optical coherence tomography is a catheter-based system that can be used to assess vascular morphology, vessel lumen and plaque components in vascular lesions. OCT utilizes near-infrared electromagnetic radiation light for the cross-sectional visualization of the vessel wall at the microscopic level. It enables detailed resolution of vessel architecture and precise characterization of plaque architecture. Quantification of macrophages within the plaque is also possible. These capabilities allow precise identification of the most clinically...

Article Overview
This article covers an October 1, 2009 Coding Clinic update on intravascular imaging terminology for optical coherence tomography (OCT). It is relevant to coders, coding educators, and clinical documentation teams working with vascular imaging and procedure classification. The discussion focuses on the scope of the new OCT-related codes, how they relate to intravascular spectroscopy, and the distinction between OCT and other imaging technologies.
Why This Topic Matters
It helps readers understand a coding update affecting vascular imaging documentation and code selection within the ICD procedure code framework.
Article Sections
-
Overview of Optical Coherence Tomography
Introduces OCT as a vascular imaging technology and summarizes the general clinical purpose of the article. It also places the update in the context of procedure classification changes effective in 2009.
-
How OCT Imaging Is Performed and Used
Describes the broad imaging approach, equipment, and vessel visualization concepts associated with OCT. It outlines the general kinds of vascular findings OCT can help assess without giving coding guidance.
-
Comparison With Other Imaging Modalities
Explains how OCT differs from coronary angiography and intravascular spectroscopy at a high level. The section emphasizes that similar delivery methods can still involve different imaging technologies.
-
Coding Clinic Update and New Procedure Codes
Presents the Coding Clinic notice and the related updates to intravascular imaging terminology. It identifies the newly created and revised procedure code entries discussed in the article.
