Pedicle Latissimus Myocutaneous Flap with Placement of Breast Tissue Expanders

A patient, who is status post bilateral mastectomy, is admitted for right pedicle latissimus myocutaneous flap and placement of bilateral tissue expanders. On the right side, latissimus was raised off its insertions and pushed up into the axilla. The skin was pushed down firmly over the muscle and closed with sutures. Skin flaps were raised and a tunnel was dissected to allow the pulling of the latissimus flap into the chest. A tissue expander was placed underneath the latissimus on top of the pectoralis muscle and the rest of the latissimus was closed over the tissue expander. On the left side, the pectoralis muscle was split and dissected out, the tissue expander was placed and the muscle was closed over the top with sutures.
