Pressure Wire Evaluation with Coronary Angiocardiography

The patient is status post coronary artery stenting of the left main, proximal left anterior descending artery as well as the mid-right and distal right coronary arteries. She developed recurrent angina and was admitted to the hospital. A diagnostic heart catheterization was performed and demonstrated significant disease in the circumflex marginal system with widely patent stents in the left anterior descending artery, left main and right coronary arteries. Two days later, the patient returned to the cardiac catheterization laboratory and underwent coronary angiography with Radi pressure wire assessment. During the procedure, a Radi pressure wire was placed in the mid-left anterior descending artery. Fractional flow reserve (FFR) was measured; however, no intervention was done on the vessel. Normally, pressure wire assessment is carried out in conjunction with balloon angioplasty, coronary stenting and/or cardiac catheterization. In this case, no interventional procedure was carried out. How should the pressure wire evaluation be coded? ...

Article Overview

This premium article explains a cardiac catheterization scenario involving coronary angiography and pressure-wire assessment after prior stent placement. It is aimed at coding and billing professionals who need to understand how the documented procedures are framed, what categories of cardiac guidance are involved, and what issues arise when a physiologic assessment is performed without an accompanying intervention. The article focuses on the clinical context, the procedure components, and the coding considerations associated with the encounter.

Why This Topic Matters

Cardiac catheterization cases often involve multiple overlapping services, and accurate classification depends on understanding the exact procedures performed and their relationship to one another. This article is relevant to professionals reviewing coronary imaging and physiologic assessment documentation in complex post-stent patients.

What You Will Learn

  • What clinical context surrounds the documented evaluation of recurrent angina
  • Which procedural components are present in the encounter
  • Why the relationship between diagnostic and interventional services matters for coding review
