AHA Coding Clinic® for ICD-10-CM and ICD-10-PCS - 2022 Issue 1; Ask the Editor
Procedures Performed on a Continuous Vessel, ICD-10-PCS Guideline B4.1c
According to the updated guideline B4.1c, if a carotid endarterectomy is performed on a single continuous lesion involving both the common carotid artery and internal carotid artery, the common carotid artery would be the only body part coded, because it is closest to the heart. We are concerned that data involving carotid endarterectomies would be skewed and procedures performed on both the common carotid and internal carotid arteries would not reflect the complexity involved in the surgery. We are requesting that Coding Clinic clarify this issue. ...

Article Overview
This article addresses an ICD-10-PCS guideline update involving procedures performed on a continuous vessel, with emphasis on carotid endarterectomy scenarios and how vessel documentation influences coding. It is relevant to inpatient coders, HIM professionals, CDI specialists, and others who work with vascular surgery documentation and ICD-10-PCS official guidance. The article explains the scope of the guideline discussion, the coding concern raised, and the distinction between a single continuous lesion and separately documented vessel involvement.
Why This Topic Matters
Accurate reporting of vascular procedures depends on understanding how the official ICD-10-PCS guidance applies when more than one vessel segment is involved. This topic matters for consistent data capture, procedural reporting, and review of documentation in carotid surgery cases.
Article Sections
-
Continuous vessel coding guidance
Introduces the ICD-10-PCS guideline update and the general issue of coding procedures performed on a continuous vessel when more than one arterial segment is involved.
-
Request for clarification
Summarizes the concern raised about how the guideline may affect reporting and data interpretation for carotid endarterectomy cases.
-
Documentation of separate lesions
Describes the distinction between a single continuous lesion and documentation that identifies separate lesions in separate vessels, within the broader vascular coding context.
