Removal of Fat Necrosis from Retroperitoneum and Space of Retzius
A woman with a pelvic abscess and pancreatic head necrosis presented for surgery. A flank incision was made with division into the retroperitoneum. Malodorous semi-solid material typical of fat necrosis was found. The area of the retroperitoneum up toward the head of the pancreas was gently debrided with fingers and sponge stick. A resectional debridement was carried out until all remaining necrotic fat and tissue were removed. For the pelvic abscess, a lower midline incision was made near the symphysis pubis until the abscess cavity was entered. This too looked like fat necrosis in the space of Retzius.
